The original death record was recorded as Patrick Callahan in the North Adams City Hall record book. The 1879 record book is closed to the public but The town clerk produced a deposition signed in 1907 correcting the orginal death record changing the name from Callahan to Cullinan.

Note 1 Patrick's marriage and death certificates showed the name Callahan. In the North Adams, Mass. City Directories for 1890 to 1893 there are eight Callahan's including a Mrs. Catherine Callahan. It is small number of People which would indicate that they are all related therefore giving the possibility that Patrick Cullinan was in North Adams, Mass with his mother and several siblings.
Note 2 Died from TNT explosion, while employed with the Boston and Maine Railroad, during the construction of the Hosiac Tunnel, North Adams, MA.
